Bring your student athlete for a sport specific screening by our RVPT Polk City team to prevent injuries before or during your athlete’s season. Following the screen, you can head over to North Polk Family Medicine to receive a comprehensive physical provided by Dr. Doug Layton and Jen Blaylock, ARNP.
